Abstract Extracellular matrix (ECM) is characterized as widespread, abundant, and pluripotent. Among ECM members, collagen widely accepted one of the most prominent components for its essential structural property that can provide a scaffold other rich biological functions, which has been extensively used in tissue engineering. Emerging evidence shown balance degradation remodeling vital to regulations maternal–fetal interface including menstrual cycling, decidualization, embryo implantation pregnancy maintenance. Moreover, disorders these events may eventually lead failure pregnancy. Although improvement assisted conception culture technologies bring hope many infertile couples, some unfavorable outcomes, such recurrent (RIF), loss (RPL) or miscarriage (RM), keep troubling clinicians patients. Recently, vitro three-dimensional (3D) model mimicking microenvironment developed investigate physiological pathological conditions The progress this technology based on clarifying role endometrium interaction between conceptus. Focusing collagen, present review summarized regulation normal menstruation, receptivity unsatisfying occurring infertility treatments, well application therapeutic approaches improve outcomes. More investigations about focusing with mesenchymal stem cells local immunoregulation inspire new thoughts advancements clinical treatments. Graphical abstract
